Dog / Puppy Application

Please complete the dog/puppy application and submit it for review. To view the animals, you must submit an application in advance. Staff will contact adopters to schedule a visit after the application is reviewed.Please see our Adoption Procedure for additional information regarding the adoption process.

Once an adoption application is approved, animals must be adopted within 48 hours (with some exceptions due to medical or surgical concerns).

Please contact LCHS at 715-536-3459 with any questions regarding the application or procedure. LCHS cannot guarantee same day adoptions. Please submit applications in advance because processing may take 1-2 business days.

Please make sure applications that are being submitted through email are word doc or PDFs. We do not have the capability to open any other formates.

Please include your drivers license at the time of submition as well.

Cat / Kitten Application

Barn Cat Application

For a cat not suited to household living, barn life might be his/her best option for receiving adequate food and shelter. If you have barns or outbuilding, your property might be the perfect place for these semi-socialized cats. They thrive in an outdoor environment but need the same basic care as a house cat, and some may become more socialized with patience and understanding.

Barn (Outdoor Homes) are required to provide fresh food and water daily, as well as shelter for the cats to escape the elements. Most cats in the barn cat program are adults and are not friendly with people.

You can upload your filled out application form below, email it to us at manager@furrypets.com, or you can print it out and bring it in to the Humane Society during our normal business hours.
